Welcome to Meadows

Meadows Nursing and Rehabilitation Center is a non-profit, faith-based, Continuing Care Retirement Community (CCRC) located in Dallas, PA. Since our beginning nearly 30 years ago, we have placed particular emphasis on creating an atmosphere of fellowship and caring—in addition to providing a high level of skilled nursing services—in an effort to meet the social, physical and spiritual needs of our residents.

Our philosophy is centered on providing excellent nursing care, along with comfort, respect and dignity to all residents, and our goal is to demonstrate that dignity in a charitable way. In addition to 24-hour skilled nursing care, we offer an aggressive program of rehabilitative therapy to keep our residents actively progressing. We provide physical, occupational and speech therapy, along with restorative nursing to those who need to improve or maintain their current condition.

Licensed as a nursing care facility by the Pennsylvania Department of Health, Meadows has 130 beds and is fully staffed by Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants, with a Medical Director on staff.
